Source of Wealth Form and Declaration Template
Create an editable wealth history with source references, a client declaration and an evidence-gap list.
Prepare the wealth history
Record How the Wealth Accumulated
A source of wealth form records how a person or entity accumulated wealth over time. Enter each material source separately, with its period and amount or range.
Use the explanation to distinguish gross earnings, retained savings and proceeds that were reinvested. Amounts remain as you enter them. The form does not add them together, because sources can overlap or use different currencies.
The fictional example records employment savings of $180,000 to $210,000 and an inheritance of $95,000. Its evidence gaps remain visible in the draft.
For a transaction-specific enquiry, use the source of funds and source of wealth guide to decide which history and payment records to collect.
Attach an Evidence Schedule
Each source has space for supporting records, their date and a source reference. List additional documents on separate lines and include their dates beside their descriptions.
Use the follow-up field for missing documents or inconsistencies. Blank record descriptions and references appear in the draft’s unresolved-gap list.
AUSTRAC’s source of wealth guidance distinguishes the underlying origin from the account holding the money. A bank credit alone may leave that origin unexplained.
Match salary claims to employment and tax records, business wealth to ownership and financial records, and asset sales to contracts and settlement records. Gifts and inheritances may need donor records or estate and probate documents.
Prepare the Declaration for Review
The output combines the supplied narrative, a source-by-source evidence schedule and declarant details. Review the wording and gaps before the declarant signs it.
This client declaration records the client’s account. It does not certify that account or determine whether a reporting entity’s due-diligence requirements are met.
If the recipient requires its own form, transfer the reviewed information into that form. If a statutory declaration is required, use the applicable jurisdiction’s approved process.
The Attorney-General’s statutory declaration guidance explains that Commonwealth, state and territory requirements differ. This tool produces a general client narrative.
Keep the Original Records Separate
The form runs in your browser. It sends no entered information or documents to FinTrace. Reloading or Clear form removes the current entries.
Download text for editing or printing in your document editor. Save form downloads a JSON copy containing the fields and edited draft. Reopen that file here to continue the review.
Changing a field rebuilds the draft and replaces manual edits. Download your edited text before changing the particulars.
Retain original bank statements and supporting documents separately. The form references those records and leaves their contents unchanged. Store downloaded copies under your matter’s access controls.
